New Patients
We are currently accepting new patients at all of our locations.
If you are a new patient you have multiple options for completing the new patient paperwork:
- In office completion: Please arrive 15 minutes prior to your scheduled appointment time.
- Patient Portal: Complete and update your information through Patient Portal
- Home print: Download and complete documents to bring with you to your appointment.
Payment Policy
Co-pays and co-insurance rates are determined by your individual insurance plan and we are contractually obligated to collect these from you. It is our policy to collect co-pays and any outstanding account balances at the time of service.
Self-Pay Patients
We charge $150 plus tax for new patients and for patients with new conditions, and $90 plus tax for follow-up appointments. This fee includes the office visit and minor treatments and procedures performed during the visit. Payment is due at the time of service.
Cosmetic procedures are not covered by health insurance. Our staff will go over the charges with you prior to performing any services. Payment is due at the time of service.
Online Payment
Looking for a quick and easy way to pay your bill?
Quick Pay allows you to pay your bill online with a credit card or e-check without having to login.
Telehealth
Telehealth allows you to have a real-time audio-visual appointment with your provider from the safety of your home. Most insurance plans now cover tele-health appointments and we offer these to both new and existing patients for select visit types.
No Show Policy
Please provide us with a 24-hour advance notice for any cancellation. No-shows may be charged a $40 cancellation fee. We understand that sometimes you may be delayed; however, please note that late arrivals may require rescheduling.

Many insurance plans require you to obtain a referral from your primary care provider prior to making an appointment with us. Once we have received and reviewed the referral, our staff will contact you to schedule an appointment.
It is important to ask your insurance carrier if any referral, pre-authorization, or pre-certification is necessary before you schedule your appointment with us.
